Training Forms & Background Check Instructions

Thank you for registering for a training. Please send the following documents to ciera@imaginela.org before attending training:

  • A scanned copy of your driver’s license and insurance card. We need this for a DMV background check.
  • A completed signed, mentor agreement form
  • A completed Personal Information Form
  • Receipt of your Live Scan Background Check (see below for instructions)

It is the policy of Imagine LA Program that each Mentor applicant completes a screening process. Part of this screening process includes completing a Live Scan background check on criminal history through Live Scan state & federal criminal history, sexual offender registry. Checks must be performed in all states resided in as an adult. 

We value your time and participation in our program thus far.  To show our appreciation, if you complete your Live Scan background check before, on the day of, or up to 24 hours after Mentorship Training, we will reimburse you up to $40  for the Live Scan fees once we have identified a family match. Please remember to submit a copy of your receipt to us
